Report due on series of lab mistakes

A report on a disturbing series of laboratory errors involving biopsy samples is due to be released either today or tomorrow.

A Ministry of Health spokesman said the report was written by an expert panel.

The panel's investigation started in June, after revelations three women had mastectomies after incorrect diagnoses, including one this year in Dunedin when two breast biopsy specimens were swapped.

It was also revealed a woman had part of her jaw cut away after a specimen mix-up at the University of Otago dental hospital in Dunedin.

Health Ministry chief medical officer Dr Don Mackie headed the 10-member panel, which included senior consultant pathologist and clinical director of South Australia's breast-screening programme, Associate Prof Gelareh Farshid.

The report had been given to Health Minister Tony Ryall and Associate Health Minister Jo Goodhew, the spokesman said.

The Southern District Health Board filed its own report to the expert panel on the mix-up at Southern Community Laboratories that resulted in one of the wrong mastectomies.

The Otago Daily Times has requested a copy of that report.
