Police said the 72-year-old female from Middlemarch was towing a trailer and on the wrong side of the road when the crash happened this afternoon near the Silverstream Bridge.
She drove directly into the path of a 73-year-old female who was coming over the bridge, Senior Sergeant Anthony Bond said.
Emergency services responded to the crash in Riccarton Rd West at 2.31pm.
Hato Hone St John said it transported two patients to Dunedin Hospital, one in a serious condition and the second in a moderate condition.
